You will need to:

1. visit one of our Customer Service Centres so that our staff can assist in completing an application form on your behalf

2. provide:

• a signed letter explaining your circumstances (reason(s) why you are only now making the Claim)

• your pension card for verification purposes

• any other supporting documentation. 

Your claim will be sent away to the relevant State Government Department for processing and you will be advised of the outcome in due course.
