Nominate a driver in relation to an infringement
We need to know if someone else was in possession of your vehicle at the time the infringement was issued.
There are 3 parts to this process:
Part 1
To nominate the driver of your vehicle at the time of an infringement being issued you must complete a Nomination Statement.
To obtain a Nomination Statement please contact 03 5272 4090 and request document to be mailed out to you.
On this form you must tell us:
- Your infringement number
- Your name and mailing address details
- Your company details if relevant
Part 2
In the second part of the form the registered owner or company authorised officer is required to provide the following information in relation to the driver of the vehicle at the time of the offence:
- name
- mailing address details
- drivers licence details
- date of birth.
Part 3
Finally the form must be signed and dated by you as the nominee and returned to us by:

No - all communication with the Internal Ombudsman needs to be in writing. The Internal Ombudsman will return any correspondence in letter form as soon as possible usually within 21 to 28 working days.

If you are not satisfied with the response from the Internal Ombudsman you should consider the other options you have which will be set out in the decision letter. This includes the option to take the matter to Court.

When your appeal has been logged to the infringement correspondence will be posted to you acknowledging receipt of the appeal and advising that the infringement has been placed on hold to be reviewed. You are not required to pay the infringement at this time. The review process may take a few weeks and at the completion of the review correspondence will be sent to you advising the outcome. If your appeal is unsuccessful a new due date for payment will be provided.
