Request a copy of a Food or Health Registration Certificate
We can provide you with a copy of your Food or Health Registration Certificate.

The Certificate of Registration is on the reverse side of the renewal notice which are emailed in November each year for the coming calendar year.

If you cannot find your registration certificate you can request a reprint.

A copy of the Certificate can only be issued to the authorised proprietor of the Food or Health business.

When you request a reprint of your certificate you will need to:

  • show that you are the registered proprietor

  • have the licence number related to your registration

  • advise us of the business name and trading address.

If the information you provide cannot be verified we may not be able to send you a copy of the certificate.

Please note: there may be an administrative fee associated with this request.
